This is only for players who played in Fall of 2011 and you must show up in person to register your child.
(exception is Munchkin's moving up to league you will need to fill out the full registration form)
Parents, please bring your completed form to the Soccer Coordinator during registration where you will be asked to review last years medical/contact form to make any changes and your all set !

PLEASE NOTE: ** DUE TO THE OVERWHELMING TURNOUT OF THE TODDLER PROGRAM LAST SEASON A CAP WILL BE PUT ON REGISTRATIONS ONCE WE ARE ABLE TO SCHEDULE ALL OF THE COACHES. IF ADDITIONAL COACHES BECOME AVAILABLE WE WILL INCREASE THE AMOUNT OF PLAYERS. WE WILL CONTACT YOU. MT. ARLINGTON RESIDENTS AS WELL AS PLAYERS THAT PLAYED IN SPRING 2011 WILL BE CONSIDERED FIRST. Proof of Birth Required for new players. Please bring copy of your birth certificate for us to visually check. We do not need to keep a copy. |
